Everything you need to know about receiving your Flex card, getting a replacement, and updating your mailing address.
Receiving Your Card
Your BCBS-AZ Health Choice Pathway Flex card is mailed to you automatically -- you do not need to request one. Cards can take up to 14 business days to arrive from the date they were sent.
- Cards are shipped via USPS (standard) or UPS (expedited).
- Cards are mailed from: PO Box 7273, Sioux Falls, SD 57117-7273.
- Your card arrives pre-activated -- there is no activation step required. You can start using it as soon as you receive it.
If it's been more than 14 business days and you haven't received your card, submit a request and we'll check on it.
While You're Waiting for Your Card
You don't have to wait for your physical card to start using your benefits. You can:
- Place orders through your portal at bcbsazflex.com and have items delivered to your home.
- Call us at 888-454-1423 and we'll place an order for you over the phone.
I Never Received My Card
If your card hasn't arrived after 14 business days:
- Submit a follow up request from our team
- We'll verify your mailing address is correct.
- If the address needs to be updated, we'll make the change.
- We'll send a replacement card to the correct address.
My Card Is Lost or Stolen
If your card is lost or stolen:
- Submit a request right away.
- We'll freeze your current card to prevent unauthorized use.
- We'll issue a new card with a new card number.
- Your new card will arrive in 5-10 business days (standard) or 1-3 business days (expedited, if requested).
- If you find your old card after a replacement has been sent, please destroy the old card.
My Card Is Damaged
If your card is physically damaged but you still have it:
- Submit a request
- We'll send a replacement card with the same card number.
- Please destroy the damaged card once your replacement arrives.
I Need to Update My Mailing Address
If your address has changed or was incorrect when your card was sent:
- Submit a request
- We'll update your address in our system.
- We'll reissue your card to the new address.
Important: If you live in an apartment or unit, make sure to provide your full address including the unit number so your card is delivered correctly.
Can I Get My Card Faster?
If you need a replacement card quickly, you can request expedited shipping when you call us. Expedited cards typically arrive in 1-3 business days. Please note that we do not offer expedited shipping unless it is specifically requested.
I'm Not Sure Which Card to Use
If you've had a card in a previous year, your old card will not work for the current year. Always use the most recent card you've received. If you're not sure which card is current, submit a request and we can confirm using the last 4 digits of the card number.
I See a Charge I Didn't Make (Fraud)
If you see a transaction on your account that you did not make:
- Submit a request right away and let us know.
- We'll freeze your card to prevent further unauthorized charges.
- If you're calling after business hours, we'll freeze the card and ask you to call the fraud team back during their hours (available Monday-Friday, 8:00 AM - 8:45 PM EST).
Please be prepared to share the total dollar amount and date of the unauthorized transaction.
